 This is our packet that we give to all of our new enrollments.  You can print this off, fill out the medical release form, and bring it in at the time of your enrollment!

Each cheerleader must read, understand, fill out, sign, and hand in the entire copy of this cheer constitution to Amber to keep on file for the 2010-2011 cheer season.  A new one must be filled out each year.

Before starting any private at Excalibur Athletics you must read, sign, and turn this agreement form in to your private coach.

All Majestics Pom Dancers need to print, sign, and turn in entire packet to Amber by the first practice.
